Premium-100% Paid by District
Eligible Employees-Full Time Employees and Part Time employees with 50% FTE
Effective Date of Coverage-1st day of Month following Hire Date
Elimination Period-60 days of continuous disability
% of Covered Salary Insured-90% of monthly salary
Maximum Gross Benefit-$12,750 per month
